Therefore, breast milk is the most ideal and necessary food for babies, and breastfeeding can benefit both mothers and children.  

  However, the current situation of breastfeeding in China is not optimistic. In February 2019, the Investigation Report on Influencing Factors of Breastfeeding in China released by China Development Research Foundation showed that the exclusive breastfeeding rate in China was about 29.2% within six months, which was lower than the world average of 43%, and it was put forward by 2020 together with the China Children’s Development Program (2011-2020) and the National Nutrition Plan (2017-2030).

  The mother’s double pressure from physiology and psychology, the degree of family support and the atmosphere created by the whole society are all affecting the instinctive behavior of mother and baby. It needs the joint efforts of the whole society to give mothers one more reason to insist on breastfeeding, so that they can receive due respect and more care during breastfeeding.

  The pain of "violent breast-feeding"

  The words "blocking milk" and "breast-feeding" appear frequently in the lives of new mothers. However, some mothers don’t know that their past experience may be a "violent breast-feeding".

  Alian (a pseudonym) is a novice mother. During lactation, she has breast swelling and caking. In order to breastfeed, she left home for the first time in the second month. However, she met an unprofessional breast nurse, which made her "hurt more".

  Alian told CCTV reporter that when she first stepped into a dimly lit room, the breast nurse wearing a mask told her to lie down, and she didn’t talk much. She didn’t specifically communicate with her about her breast and psychological situation, nor did she ask her about the frequency and health of her baby’s breastfeeding. When she came up, she rubbed her breast lump with great force. "Her technique is very heavy. Three times of breast feeding made me feel terrible, which is more painful than giving birth to a child." Alian said.

  After breast-feeding, Alian was very depressed. But when she came home and saw the baby in its infancy, she turned to strengthen her confidence in breastfeeding. However, is it so painful to pass milk? Yan Liu, a community breast-feeding consultant and an internationally certified lactation consultant (IBCLC), said that if you press hard and feel pain during breast-feeding, you can initially judge that it is violent breast-feeding. Breast tissue is very superficial and fragile, especially during lactation. Manual violent massage and kneading of the breast will damage the mammary duct, which will lead to more serious breast diseases.

  "It doesn’t hurt when a real professional nurse passes milk." Alian later learned that professional breast nurses would first find out the direction of breast milk and understand the situation of breast milk, instead of blindly "violently breast-feeding". "Actually, the baby is the best ‘ Nurse ’ 。” Alian also suggested that mothers who are "painful and persistent" like her should let their babies breast-feed, find a professional and qualified breast-feeding teacher, and bravely say "no" if they encounter "violent breast-feeding".

  The common breast-feeding problems, such as physiological breast enlargement, milk deposition and mastitis, which can be effectively alleviated by improving the baby’s sucking and cold compress, have been used by merchants to make articles.

Yan Liu said that community breastfeeding support needs professional mother support groups, social workers or commercial organizations to assist mothers to receive prenatal breastfeeding education, to teach mothers to deal with common postpartum breastfeeding problems, to support mothers to discover and recognize their own and their children’s instincts, to help mothers find suitable referral channels and to give them enough psychological support and companionship.

  According to a research on a mutual support group project for breastfeeding mothers in Michigan, USA, it was found that compared with mothers who did not receive support activities from mutual support groups, mothers who received support from mutual support groups could persist in breastfeeding for at least two weeks longer, and the rate of early lactation would be increased by 22%.

  "Empowering" is not a slogan, but requires everyone’s real input. Yan Liu said that the staff of community breastfeeding support institutions need to have higher entry threshold and unified professional qualifications, and these institutions should be inspected and audited regularly. Approved institutions can be included in the list of community support organizations referred by hospitals for mothers after discharge, and local hospitals can recognize the evaluation opinions of these institutions before referring mothers. "However, such community breastfeeding support institutions are currently scarce in China."

  This year, Guangzhou is the first in China. Legislation to promote breastfeeding has achieved initial results, and it also reflects the characteristics of local legislation. In January 2019, 74 NPC deputies jointly submitted the "Proposal on Legislation to Promote Breastfeeding" at the Fourth Session of the 15th Guangzhou Municipal People’s Congress. In May, the Standing Committee of Guangzhou Municipal People’s Congress voted to pass relevant decisions, and the bill officially entered the legislative process. In June, the Social Construction Committee of Guangzhou Municipal People’s Congress began to review the draft articles, solicited opinions from many parties and conducted in-depth legislative research and scientific argumentation. On this basis, the Regulations on the Promotion of Breastfeeding in Guangzhou (Draft Revision Proposal) was formed. At the end of July, the "Draft Revision Proposal" passed the first trial, and the second trial is planned for September.

  The "Proposal for Revision of the Draft" also stipulates the construction, popularization of science, scientific research and management of maternal and infant rooms, among which the Guangzhou Municipal Health Administration Department is the supervision and management department for breast-feeding promotion, the Guangzhou Municipal People’s Government provides the necessary funds for the construction, operation and management of breast milk banks, and the managers of breast milk banks give certain nutritional subsidies to donors to solve the problem of difficult operation of breast milk banks.

  "The whole law hopes to form a breastfeeding support system with the participation of the whole society by mobilizing all resources and strength of government departments, medical institutions, employers, society and families." Lei Jianwei said that the most effective and influential way of policy advocacy is legislation. Breastfeeding legislation can have a lasting impact on relevant people in this field in Guangzhou.
